It should be noted that the sheet in connection with disability has the right to grant, not every medical facility and doctor working in it. To do this, first of all, the organization must have a license to conduct medical practice. Issuance of medical certificates are authorized to:

• Treating doctors of hospitals and outpatient care centers (clinics).
• Paramedics.
• Dentists.
