How to fill out a questionnaire on a Finnish visa

To visit Finland, citizens of the Russian Federation need to have permission to enter this country. It can be either a Schengen multivisa, or a separate visa for entry into Finland. For its design, the following documents are needed: passport (which is valid for another 3 months after the end of the trip), color photo 3.5x4.5 cm, medical insurance and questionnaire. How correctly fill out the questionnaire on the Finnish visa?

All items are filled in English, printed letters. Information that is spelled out in a passport (name, surname, place of birth, etc.) must be introduced into the questionnaire "Letter to the letter". In the ducting form, they are not set anywhere. If at any point it is necessary to give a negative answer - write "no" or put a tick in the designated place.

In paragraph 2, women prescribe their maiden name, men write "no". Farther:

p. 4 - date, month, year of birth - all numbers;
clauses that were born until 1991 in the Country and Place of Birth, should write "Soviet Union" or "USSR", after 1991 - "Russian Federation";
clause 7 - citizenship "Russia" for Russian citizens or indicate another country, if you were not born in this country;
p. 10 - leave empty (filled only in the questionnaire for a child).

Counts 11-12 - FULL NAME of your parents - write them down in English; p. 14-17 - discharged from the passport; Count 18 - If you are a citizen of Russia - write "no".

The following items fill in this way:

p. 22 - the first in the list of countries in which you plan to go, should stand "Finland";
n. 25 - For how long the visa is needed (up to 90 days);
clause 26-28 is enough to write the term of the last Schengen visa. If it was not, then we write "no" and place for dates are empty;
paragraph 29-30 - the date of opening of the visa (as a rule, 10 days later the date of filling in the questionnaire), the date of the end of the visa is 12 or 6 months and minus one day;
p. 31 - who invited usually writes the address of the hotel;
clause 32 - Filled only for those who are going on work, and tourists leave this item unfilled;
clause 33 - What way do you cross the border of Finland: "by air", "by train", "by car" - by plane, train or car.

After filling out all the points of the questionnaire, put the date and signature. It must be put by the applicant personally. The signature supplied by a third-party person can serve as a basis in refusing to issue a visa.
